The tuner is only for over the air reception. Note that satellite and cable often do not carry 100% of available local OTA broadcasts. You would have to get an HD box to pick up your HD channels. The difference is that tv's with ATSC tuners built right into it is able to pick up your locals channels over the air waves through your HD antannae. Usually there are only a few channels. I would suggest that you speak to your local electronics store and ask them more about the channels that you can receive through the ATSC tuner and an HD antannae. Chances are there are not a lot of channels. And to get most of the popular channels you would have to subscribe to HD Satellite.
